Who can make a referral?
Patients can be referred to Hospice Taranaki by their GP, hospital doctor, or medical specialist, or self referral is possible.
However as it is a community organisation, anyone - be it the patient, family member or a health professional - should feel free to contact the hospice if they are concerned that any needs of someone living with a life limiting illness are not being met. It is important that before making this call that you do have that patient's permission.
Phone Te Rangimarie 06 753 7830 to find out more.
Download a Referral Form click here - you must print this form off, fill-in the required information by ink, and then you must email: firstname.lastname@example.org OR we can accept faxes if necessary at 06 753 7806.
An assessment by a member of the Hospice palliative care team may then be required.
